What happens after you submit your information
Upon receipt of the information form above, a Preliminary Application Form will be sent to you along with fees and next steps.
Please note that no vital records nor payment are needed at this time. Once we can confirm that your line is plausible, we will help you with your application process and documenting your lineage. Minimum requirements will be that you are able to provide copies of your birth, marriage, and death certificates for yourself (and your spouse if married or previously married), your parents, and your grandparents (if available). We will be assisting you with the process of documenting previous generations as well. Each document must connect the next generation.
If you have any general questions, you can also email our State Historian.
